Can hypothyroidism or diabetes cause carpal tunnel syndrome?
Diabetes and hypothyroidism (an underactive thyroid) are both linked to carpal tunnel syndrome, but the studies show an association, not proof that either one causes it. The diabetes link shrinks once other factors are counted, and the hypothyroidism link was not significant overall in a 2025 study, though it was higher in people under 50. If you have either condition and persistent numbness, get it checked.

Both conditions are associated with carpal tunnel syndrome, and the association is real but modest. Neither has been shown to cause it directly. The sections below set out what the studies measured, why the figures change with the statistics used, and what to do if you have either condition and numbness in your hand.

Diabetes is linked to carpal tunnel, and the link shrinks after adjustment
Carpal tunnel syndrome is pressure on the median nerve at the wrist. Across 25 studies that reported unadjusted figures, diabetes had a pooled odds ratio of 1.97 for carpal tunnel syndrome or carpal tunnel release surgery 1. The same review, using 18 studies that controlled for other factors, found the figure fell to 1.69 2.
An odds ratio compares odds, not chances, so it is not a direct percentage increase in risk. The drop from 1.97 to 1.69 matters. Diabetes often comes with body weight and other factors that also affect the nerve, so part of the first figure reflects those. The adjusted figure is the better guide to what diabetes adds on its own.
Hypothyroidism shows a modest link that was strongest under 50
An underactive thyroid, called hypothyroidism, has a similar pattern. In studies that adjusted for some confounding factors, the effect size fell to 1.44 3. That is the more reliable figure, because adjustment removes part of the effect of other factors that come with the condition.
A 2025 retrospective study looked at 480 patients referred for upper limb nerve testing. After adjusting for age and sex, the overall association between hypothyroidism and carpal tunnel syndrome was not significant 4. In the subgroup under 50, carpal tunnel was more common among patients with hypothyroidism, with an odds ratio of 2.59 5.
The subgroup is small, and the overall result was not significant. It is a lead for further study, not a settled finding. The two results together suggest the link may depend on age, but the study cannot establish that.
Why thyroid disease might affect the tunnel is still a hypothesis
The proposed explanations center on the tissues around the nerve. An underactive thyroid can change connective tissue, and that could thicken the lining of the tunnel and press on the median nerve. These are hypotheses. The studies cited here measure how often the two conditions appear together. They do not measure pressure inside the tunnel of people with hypothyroidism.
Amyloid deposits are a rarer cause, and they matter most after 50
Carpal tunnel syndrome can also be caused by deposits of misfolded proteins called amyloid, and it is often an early sign of systemic amyloidosis 6. The condition is uncommon, but it is worth knowing about because it can come years before heart problems.
In one study of people having surgery for idiopathic carpal tunnel syndrome, amyloidosis was found on biopsy in 10.2 percent of men over 50 and women over 60. All of those positive patients had symptoms in both hands 7. A review of transthyretin amyloidosis reports that carpal tunnel syndrome can develop five to ten years before cardiac amyloidosis 8. The timing comes from other reports and case series, so treat it as an approximate interval.
The biopsy figure comes from people over 50 and women over 60, so amyloid is a less likely explanation for numbness in a younger person. It is still worth raising with a clinician if numbness is in both hands and you are older.
What to do if you have diabetes, thyroid disease or both
- Tell your clinician about the numbness early. Note which fingers are affected, whether it wakes you, and whether both hands are involved.
- Ask whether your blood sugar and thyroid levels are being checked as part of your usual care, and whether they are well controlled.
- Do not expect treating the thyroid or blood sugar to settle the nerve by itself. The studies above do not show that treating these conditions reverses carpal tunnel syndrome. Ask your clinician how to manage the nerve directly.
- Do not wait for weakness to appear. Persistent numbness needs a clinical check. The options from splints to surgery are covered in our guide to carpal tunnel treatment.
