Dictation for RSI and Carpal Tunnel: Type Less by Voice
Dictation is the one change that removes keystrokes instead of rearranging them, so it can take much of the drafting and messaging off sore hands. Voice commands can cover much of the editing and pointing, but they take practice. No controlled trial we found has tested dictation in people with RSI or carpal tunnel syndrome, so use it alongside medical care and good ergonomics, not as a treatment.

If your hands ache when you type, dictation removes keystrokes instead of rearranging them. You speak the words and the software types them, so your fingers and wrists do no work for that text. It will not cure carpal tunnel syndrome or repetitive strain injury (RSI), and the research on dictation for those conditions is thin. It can, however, take much of the drafting, email and messaging off sore hands while you get medical care.
The plan has three parts. Drafts, messages and email move to voice fairly easily. Editing and pointing need their own plan, built from voice commands, keyboard shortcuts and regular breaks. And the free dictation already on your Mac, Windows PC or phone is enough to start. Stop and see a clinician if your symptoms get worse or do not go away.
Dictation removes keystrokes, while splints and ergonomic gear only change how you make them
Carpal tunnel syndrome involves pressure on the median nerve at the wrist, and the glossary entry on the median nerve explains that nerve. Repetitive strain injury is a broader term for pain from repeated movements, covered in the glossary entry on repetitive strain injury.
What causes it at work is harder to pin down. The US National Library of Medicine says studies have not proved that typing, using a mouse or other repeated movements cause carpal tunnel syndrome 1. That does not make keyboard work harmless. The UK Health and Safety Executive (HSE) lists carrying out a task for a long time without suitable rest breaks as a risk factor for upper limb disorders 2. The useful question is therefore which change reduces the load on your hands.
Splints, desk setup and ergonomic equipment change how your hands make the keystrokes you still need. The evidence for them is limited. A 2022 Cochrane review found insufficient trial evidence to say whether ergonomic positioning or equipment helps or harms people being treated for carpal tunnel syndrome 3. For splints, the NHS advises wearing a wrist splint at night, and says it may take up to 6 weeks before symptoms start to improve 4.
Dictation is different because it removes keystrokes. In a controlled study on an iPhone, English text entered by speech reached 153 words per minute, against 52 words per minute on the built-in keyboard 5 6. Put in time, 1,000 words take about 6.5 minutes by speech 7 and about 19.2 minutes on the keyboard 8. Those figures come from short messages in a laboratory, and the study's authors say results outside the lab still need study 9. Treat the ratio as a guide, not a promise.
| Option | What it changes | What the evidence shows |
|---|---|---|
| Dictation | Replaces the keystrokes for the text you say | Faster for entering short messages in one controlled phone study; no trial in RSI or carpal tunnel |
| Night wrist splint | Rests the wrist while you sleep | The NHS says it may take up to 6 weeks to help 4 |
| Ergonomic keyboard or positioning | Changes wrist posture while you type | Cochrane: insufficient trial evidence |
| Regular short breaks | Interrupts the repetition | Cochrane: very low-quality evidence of effect |
No controlled trial we could find compares dictation with typing in people with RSI or carpal tunnel syndrome. Our search found vendor case studies, forum reports, and an accommodation listing from the US Job Accommodation Network, but no randomized study. The guide to dictation as a workplace accommodation covers how to ask for it at work.
Whether you need to stop typing altogether is a separate question, and it is covered in should you stop typing with RSI?.
Most writing can move to your voice, but editing and pointing need their own plan
Drafting moves to voice most easily: a first draft, a long email, a reply to a message. Speaking the words is usually quick to pick up. Editing is where dictation slows down. In the phone study, speech produced fewer errors while entering text, but the final text kept slightly more of them. Speech had a corrected error rate of 5.30 percent against 11.22 percent for the keyboard, while its uncorrected error rate was 1.30 percent against 0.79 percent 10.
| Measure in the phone study | Speech | Phone keyboard |
|---|---|---|
| Corrected error rate during entry | 5.30% 10 | 11.22% 10 |
| Uncorrected errors in the final text | 1.30% 10 | 0.79% 10 |
Corrections are where a sore hand can get pulled back into the work. Every fix made with the keyboard is a run of keystrokes, and fixing errors by voice is slow at first. Kraal and colleagues report that users who keep dictating for about 20 hours tend to combine spoken correction with keyboard and mouse correction 11. Practice is the way through.
A practical editing routine:
- Dictate one paragraph or a few sentences at a time, so each error is close to where you noticed it.
- Fix errors right away while the sentence is still in view, rather than leaving them for a later pass.
- Learn the three or four editing commands you use most, and keep a keyboard shortcut for the rest.
- Keep a list of names, jargon and product words that the software gets wrong, and add them to the custom vocabulary if your software has one. See the glossary entry on custom vocabulary.
Dictation is a poor fit for code and dense formatting, where you need exact symbols and precise cursor placement. Keep the keyboard for that work. For more on the common errors and how to reduce them, read the guide to dictation mistakes.
Voice commands and full voice control can replace much of the mouse and trackpad
Pointing and clicking are the other repetitive job. A 1992 study compared speech-activated commands with mouse commands in word-processing tasks. The speech commands reduced task time by an average of 18.67 percent 12, but participants made more memorization errors with speech than with the mouse 13. That study is old and used early software, so read it as evidence of direction, not as a benchmark for today's tools.
Current systems go further. On Windows, Voice Access has a grid overlay that moves the pointer by number: say a number from one to nine to move the mouse pointer to the center of that tile 14. A Mac's Voice Control can overlay a numbered grid on the screen so you can click a precise area by number 15, and the guide to Mac voice control commands and the guide to Windows Voice Access commands list the phrases for each system.
On macOS 27, Voice Control has modes, and the mode decides what your words do. In Dictation mode, which is on by default, any words that are not commands are entered as text 16. If you forget which mode is on, your commands end up as text, so check the mode before you start.
The Windows grid overlay places the pointer at the center of a tile, so fine positioning takes extra steps. In practice, expect to keep a trackpad or mouse for small targets and use voice for large moves, clicks and menus.
Dictation works best as a hybrid of voice, keyboard shortcuts and breaks
Long-term dictation users appear to mix methods rather than rely on one. The nearest evidence is the 20-hour finding: users who keep dictating tend to combine voice correction with keyboard and mouse correction 11. That is a hybrid, and it is the pattern to aim for. Keep the keyboard for the parts of the work that do not suit your voice, and move the rest.
Breaks have some evidence behind them, although it is weak. The 2018 Cochrane review found very low-quality evidence of an effect of supplementary breaks on upper limb discomfort in office workers 17. The HSE's guidance for display screen users recommends short breaks often rather than longer ones less often, for example 5 to 10 minutes every hour rather than 20 minutes every 2 hours 18.
Typing speed returns to its old level by weeks 2 to 3 after surgery
If you are switching because of surgery, the timeline matters. In a 2016 study of open carpal tunnel release, average typing speed on a short online test fell from 49.7 words per minute before surgery to 45.2 words per minute at 8 to 10 days after 19 20. Speed passed the pre-surgery level between weeks 2 and 3 21, and reached 53.5 words per minute at 12 weeks 22.
Speed is not the same as comfort. Kaiser Permanente advises avoiding repeated hand movements, including typing, for up to 2 weeks after surgery 23. Your surgeon's instructions come first. The guide to carpal tunnel treatment covers the options, and the question on typing after surgery covers the timeline in more detail.
A staged switch you can try
The steps below are our suggestion, built from the evidence above. No trial has tested this exact sequence.
- Week one: dictate messages and replies only, and do every edit on the keyboard.
- Week two: move first drafts of documents and email to voice. Keep editing on the keyboard.
- Week three: add voice commands for the edits you repeat most, and keep a keyboard shortcut for the rest.
- Ongoing: set a break timer for 5 to 10 minutes every hour 18, and stop a session when your hand or voice tells you to.
The free built-in dictation on Mac, Windows and phones is enough to start
You do not need to buy anything to test dictation. Each major platform includes it, and the table below shows how to start on each and what to check before you rely on it.
| Platform | How to start | Things to check |
|---|---|---|
| Mac (macOS 27) | Press the Microphone key (if your keyboard has one), press the Dictation shortcut, or choose Edit > Start Dictation 24 | Check the Keyboard settings if you want a double press of the Fn key to start dictation 25 |
| Windows 11 | Press the Windows key + H on a hardware keyboard 26 | Voice typing needs an internet connection, a working microphone and the cursor in a text box 27. |
| iPhone (iOS 27) | In Settings, tap General, then Keyboard, and turn on Enable Dictation 28 | Read the iPhone dictation guide for the keyboard steps. |
Windows voice typing is not an offline option, so the table matters if you dictate in places without reliable internet. Separate guides cover the current steps: dictation on Mac, voice typing on Windows, and Windows dictation privacy for what leaves your device.
Yap, which we make, is an on-device dictation app for Mac and Windows. It is worth a look if the built-in tools are not enough, and you can download it to try. You do not need it to follow the plan above.
Speaking all day loads your voice, so pace it like any other repetitive work
Dictation moves the load from your hands to your voice. The voice is not endless, and the research on dictation and voice is small. A 2007 study had four groups of speakers do a speech recognition task for 2 hours 29. One of its most consistent results was a rise in pitch, a common finding in voice fatigue studies, which the authors read as a response to early voice fatigue 30. The study measured voice quality after one session in a laboratory, not long-term damage.
The US National Institute on Deafness and Other Communication Disorders advises not overusing your voice, and avoiding speaking when your voice is hoarse or tired 31. The advice is general voice care for everyone, not a study of dictation users.
Noise adds a second problem. Apple advises avoiding background noise when you dictate on a Mac 32, and a 1996 study reports that speech recognition performance degrades rapidly in ambient noise 33. Dictate somewhere quieter where you can, and see the guide to dictation and background noise for the details.
A pacing plan we suggest, not a tested limit:
- Dictate in sessions, and take the same 5 to 10 minute breaks 18 you take from typing.
- Stop when your voice feels tired, hoarse or strained, and switch to the keyboard for the rest of the day.
- Keep your speech steady and unforced rather than shouting over the microphone.
No study we found sets a safe number of dictation hours. The question on whether dictating all day can strain your voice covers what the evidence does and does not show.
Dictation supports medical treatment but does not replace a diagnosis
Dictation is a tool that works alongside treatment, not instead of it. A clinician can confirm whether your symptoms come from the wrist, the neck or somewhere else, and that decision changes what you should do with your hands.
The NHS says to see a GP if your carpal tunnel syndrome symptoms are getting worse or are not going away 34. If home treatment is not working, ask about other options, including surgery, which a specialist can discuss with you 35. The guide to carpal tunnel treatment sets out the choices.
At work, the accommodation route matters, and the guide to dictation as a workplace accommodation explains how to make a request.
This guide is general information, not advice about your hands. It does not diagnose or treat anything. Ask a clinician which tasks to change, for how long, and when to return to them.

Questions about this
- Can dictating all day strain your voice?Dictating all day can tire your voice, and long sessions without rest may make that worse, but the evidence is small and does not show lasting damage. No study we found sets a safe number of hours, so rest your voice when it feels tired or hoarse and use the keyboard for the rest of the day.
- Should you stop typing if you have RSI or carpal tunnel? The evidence does not show that you must stop typing altogether. Stop a task for a while if it makes your symptoms worse, take regular breaks, and see a clinician if numbness, weakness or pain persist or get worse.
- When can you type after carpal tunnel surgery?Your surgeon's instructions come first, but Kaiser Permanente advises avoiding repeated hand movements such as typing for up to 2 weeks after carpal tunnel release. In one study of open release, typing speed passed its pre-surgery level between weeks 2 and 3, while grip strength took 5 to 7 months to show marked recovery.
