What causes carpal tunnel syndrome in pregnancy?
Carpal tunnel syndrome in pregnancy is mainly linked to swelling. Fluid retention, hormonal changes, weight gain and posture add pressure on the median nerve inside the wrist's fixed tunnel. Fluid retention is the one factor here with a measured link, and symptoms often improve after delivery, though not always fully.

Carpal tunnel syndrome in pregnancy has no single cause. Fluid retention, hormonal changes, weight gain and posture are the factors researchers and clinicians name most often. Fluid retention is the most direct route to swelling. The carpal tunnel is a fixed space in the wrist, so when the tissues around the nerve swell, the median nerve has less room and is squeezed.
Fluid retention is the one factor with a measured link
Fluid retention is the one factor in this answer with a measured link. In a Dutch cohort of 639 pregnant women, higher fluid retention throughout pregnancy was significantly related to carpal tunnel symptoms, with an odds ratio of 1.8 1. An odds ratio of 1.8 means the odds of symptoms were about 1.8 times higher in women with more fluid retention. The study is observational, so it shows an association, not proof that fluid causes the symptoms. It also did not test whether symptoms ease when the fluid settles after birth.
The reference summary from the U.S. National Library of Medicine lists fluid retained during pregnancy among the factors that may lead to the condition 2, which matches the Dutch finding.
Hormones, weight gain and posture are the other likely drivers
A 2024 narrative review of carpal tunnel syndrome in pregnancy describes the clinical picture as primarily determined by hormonal changes, postural modifications, weight gain and fluid retention 3. The review links the hormonal changes to raised blood levels of oestrogen, aldosterone, cortisol, prolactin and glucose. It also describes the fluid retention as causing swelling, mainly because blood returns more slowly from the limbs.
The review is a summary of published work, not a trial, so it does not rank these factors against each other. The odds ratio above is the only figure in this answer that measures the effect of one factor directly. Posture is on that list too, and the typing ergonomics guide covers wrist position for everyday work.
Pregnancy symptoms are common, not a rare complication
Symptoms are common in pregnancy. In the same Dutch cohort, 34 percent of the women reported carpal tunnel symptoms during pregnancy 4. The symptoms were recorded on a questionnaire, so the figure counts symptoms, not confirmed cases on nerve testing. It still shows that symptoms are not rare in pregnancy.
It depends: who is more likely to have symptoms in pregnancy
The pregnancy explanation is most straightforward when numbness starts during pregnancy. Three situations call for a closer look:
- You already had risk factors. Diabetes is linked to carpal tunnel syndrome outside pregnancy too 5. The thyroid evidence is mixed, and a 2025 study of 480 referred patients found no statistically significant overall link 6. If they were present before pregnancy, the symptoms may be the same condition that was already developing, not a pregnancy-only one. The question of thyroid disease is covered in can hypothyroidism cause carpal tunnel.
- The little finger is numb. The little finger's feeling comes from the ulnar nerve, not the median nerve 7, so a numb little finger points away from the wrist tunnel.
- The numbness does not follow the nerve. Numbness across the whole hand, or numbness together with neck pain, can point to other causes. The guide to carpal tunnel compared with a pinched nerve in the neck sets out the difference.
Symptoms often ease after birth, but not always fully
Carpal tunnel that starts in pregnancy often improves after delivery, but not for everyone. In an Italian cohort of 45 women followed for three years, symptom scores had improved, but about half still reported symptoms at three years 8. The full answer is in does carpal tunnel go away after pregnancy.
What to do during pregnancy
- Wear a wrist splint at night if numbness wakes you. The NHS advises a wrist splint at night to relieve pressure on the nerve, and says it may take up to 6 weeks before symptoms start to improve 9.
- Reduce long periods of bent-wrist work. Rest the hands during long tasks, and keep the wrist straight when you can. Dictation can also cut the keyboard work that keeps the wrist bent, which is covered in the guide to dictation for RSI and carpal tunnel.
- Ask your midwife or obstetrician which options are safe for you. Treatment choices during pregnancy depend on your stage and your health, so ask before you take any medicine or have an injection.
- Get checked if numbness persists or weakens. If carpal tunnel is left untreated for too long, it can lead to permanent dysfunction of the hand, including loss of feeling and weakness 10. Do not wait for weakness to appear before getting checked.
