Which fingers does carpal tunnel syndrome affect?
Carpal tunnel syndrome usually affects the thumb, index finger, middle finger and the half of the ring finger that the median nerve supplies. It usually spares the little finger, because a different nerve, the ulnar nerve, supplies it. If the little finger is numb, another cause is more likely, such as the ulnar nerve at the elbow or a nerve in the neck.

Carpal tunnel syndrome usually affects the thumb, index finger, middle finger and half of the ring finger. The numbness is often felt most in those fingers, and it often wakes people at night 1. The little finger is usually spared. A numb little finger does not fit the typical pattern, and it points toward a different nerve or a different place along the arm.
Carpal tunnel numbs the thumb, the first two fingers, the middle finger and half the ring finger
The carpal tunnel is a passage at the wrist, and the median nerve runs through it. The median nerve supplies feeling to the skin of the thumb, index finger, middle finger and half of the ring finger 2. When the nerve is squeezed, those are the areas that go numb, tingle or ache, and they are the same areas the nerve serves when it is healthy.
The table below shows which nerve supplies each finger, and which fingers carpal tunnel usually involves.
| Finger | Nerve that supplies feeling | Usually affected by carpal tunnel |
|---|---|---|
| Thumb | Median nerve | Yes |
| Index finger | Median nerve | Yes |
| Middle finger | Median nerve | Yes |
| Ring finger, half nearest the middle finger | Median nerve | Yes |
| Ring finger, half nearest the little finger | Ulnar nerve | No |
| Little finger | Ulnar nerve | No |
Two rules follow from the table. Numbness in the thumb, index and middle fingers is the core sign of carpal tunnel. The ring finger is split between the two nerves, so it can be affected in part, while the little finger is usually spared.
The little finger is usually spared, so numbness there points to another cause
The little finger is supplied by the ulnar nerve, which gives sensation to the little finger and part of the ring finger, and power to the small muscles within the hand 3. Carpal tunnel syndrome is pressure on the median nerve, not the ulnar nerve 2, so the little finger is usually not involved.
The most useful alternative to check is cubital tunnel syndrome. It is compression or irritation of the ulnar nerve in a tunnel on the inside of the elbow 4. That tunnel changes shape with the elbow. In a cadaver study, its cross-sectional area shrank by 30 to 41 percent as the elbow moved from full extension to 135 degrees of flexion 5. A tingling little finger that comes on when the elbow is kept bent is therefore a reason to ask about the elbow as well as the wrist. Our guide to carpal tunnel or cubital tunnel syndrome sets out how the two differ.
Because the pattern is a clue and not a diagnosis, a clinician should check the fingers and the elbow, rather than assume the cause from which fingers are numb.
The thumb is the finger to watch for weakness and muscle loss
Numbness is the early sign, but the thumb is where the condition shows itself when it becomes more serious. The NHS lists a weak thumb or difficulty gripping among the symptoms of carpal tunnel syndrome 6. A 2024 Cochrane review says that in severe cases, skin sensation can be permanently reduced, and compression may cause muscle wasting at the base of the thumb 7.
That stage is for severe cases. Mild numbness does not usually reach it. If the thumb is getting weaker, or you start dropping things, tell a clinician. That change is a reason to act sooner rather than wait.
Numbness spread along a band of skin points to the neck
Numbness that comes from the neck follows a different pattern. In a nerve root problem in the neck, the numbness is dermatomal, which means it follows a band of skin, and the weakness is myotomal, which means it follows a group of muscles 8. A band of numbness that runs up the arm, or that does not match the median nerve's fingers, is a reason to look beyond the wrist.
Neck problems can also feel like carpal tunnel, and the two can occur together. Our guide to carpal tunnel or a pinched nerve in the neck explains the differences in more detail.
It depends on the pattern: what each combination of fingers suggests
- Thumb, index, middle and part of the ring finger, worse at night: this matches the median nerve pattern of carpal tunnel. Ask about a wrist assessment. Why carpal tunnel is worse at night explains the night-time pattern.
- Little finger and the outer half of the ring finger: this matches the ulnar nerve. Ask about the elbow as well as the wrist.
- Numbness that follows a band up the arm, with neck pain: this may be a nerve in the neck. Carpal tunnel can still be present alongside it.
- Numbness across the whole hand with no clear pattern: this needs an examination. Symptoms alone do not settle the cause, and the same complaint can come from several places.
The pattern is a starting point. Other conditions can cause numbness in the same fingers, so the pattern narrows the search rather than settling it.
What to do if your numbness is not in the usual fingers
- Write down the fingers. Note which fingers are numb, whether the numbness wakes you, and when it started.
- Check the thumb. Note any weakness, dropping things, or loss of grip strength.
- See a GP if it is getting worse or not going away. Symptoms that keep going or spread need a clinician's look, even while you try changes at home.
- Ask how the cause is decided. The question of how carpal tunnel is diagnosed explains the examination and the nerve tests.
