Is it carpal tunnel or cubital tunnel syndrome?
Numbness in the ring and little finger, with the thumb spared, points to cubital tunnel syndrome, a squeezed ulnar nerve at the inside of the elbow. Numbness in the thumb, index and middle fingers points to carpal tunnel syndrome at the wrist. Elbow pain is not among the sites the NHS Inform leaflet lists for carpal tunnel pain, so it is a reason to check the elbow, and a clinician examines both sites to be sure.

Numbness in the ring and little finger, with the thumb spared, points to cubital tunnel syndrome. That is a squeezed ulnar nerve at the inside of the elbow. Numbness in the thumb, index and middle fingers points to carpal tunnel syndrome at the wrist. Elbow pain is not among the sites the NHS Inform leaflet lists for carpal tunnel pain 1, so it is a reason to look at the elbow. A clinician examines both sites to be sure, because the two can occur together.
The finger pattern follows the nerve
Each nerve feels a fixed set of fingers. The median nerve, which runs through the carpal tunnel at the wrist, supplies feeling to the thumb, index finger, middle finger and half of the ring finger 2. Carpal tunnel numbness is therefore usually on the thumb side of the hand.
The ulnar nerve is different. It gives sensation to the little finger and part of the ring finger, and power to the small muscles of the hand 3. Numbness in the little finger, with the thumb spared, therefore points to the ulnar nerve, and the elbow is one place where that nerve can be squeezed, because it passes through a tunnel there.
Cubital tunnel is compression at the elbow
Cubital tunnel syndrome is compression or irritation of the ulnar nerve in a tunnel on the inside of the elbow 4. Bending the elbow narrows that tunnel. In a cadaver study, the cross-sectional area of the tunnel shrank by 30 to 41 percent as the elbow moved from full extension to 135 degrees of flexion 5. In a small surgical study of patients with the syndrome, the average pressure 1 cm into the tunnel was 105 mm Hg with the elbow flexed 6.
Those two studies show the mechanism, not how common the problem is.
Can carpal tunnel syndrome cause elbow pain?
Carpal tunnel syndrome is a wrist condition, and its symptoms are mostly felt in the hand. The NHS Inform leaflet on carpal tunnel lists pain in the hands, sometimes the wrist or forearm, and says pain may occur further up the arm, sometimes to the shoulder 1. The leaflet does not list the elbow as a place where carpal tunnel pain sits.
So pain that is centred at the elbow, or numbness in the little finger, makes cubital tunnel the more likely cause. Carpal tunnel can still be present alongside it. People with elbow and wrist symptoms together should have both checked, rather than assume that one explains the other.
What to check and what to change
- Map the numb fingers. Thumb, index and middle point to the wrist. Ring and little point to the elbow. Both groups together suggest that both nerves may be involved.
- Check your elbow posture. A phone held at the ear, a chair arm under the elbow, or leaning on a desk edge holds the elbow bent for long periods, which narrows the tunnel 5.
- Set the keyboard height. The UK Health and Safety Executive's standard setup puts the keyboard just below elbow height, with shoulders relaxed 7. That keeps the elbow open while you type.
- See a GP if the numbness persists, the hand weakens, or the symptoms get worse. The NHS gives that advice for carpal tunnel 8, and the same caution applies to the elbow.
For the wrist side of the picture, the carpal tunnel syndrome guide explains the nerve and the typing evidence, and the carpal tunnel treatment guide covers the options. The full set of hand and arm causes is in hand and wrist pain from typing, and the wider subject is in the accessibility and health overview. If the numbness starts in the neck, the pinched nerve comparison is the one to read next, and if the ache sits in the thumb, the de Quervain's comparison is the next one. The median nerve entry defines the nerve that carpal tunnel affects.
