Question · Dictation on Windows
What is the best medical dictation software for Windows?
No single medical dictation software for Windows is proven best. Dragon Medical One is the product this guide uses as its example: it is cloud-based, sold per user through resellers, and in a 2025 pilot its accuracy was not statistically different from a Whisper-based model. Choose on vocabulary, cost and privacy terms, and get the vendor's written HIPAA business associate terms before any patient information goes into an app.

We cannot name one best medical dictation product for Windows from the evidence we found, because the accuracy study we found is small and the prices come from resellers. This page uses Dragon Medical One as its main example, because it is the product the sources we found both price and test. What decides the choice is the clinical vocabulary you need, the total cost per clinician, and whether the vendor will sign terms that cover patient information. This page is general information, not advice for your practice, and the legal questions belong to your compliance team.
Dragon Medical One is cloud-based and sold through resellers
Philips describes Dragon Medical One as cloud-based clinical speech recognition for documenting care in the electronic patient record and beyond 1. Because it is cloud-based, the vendor's data terms matter as much as the features.
Nuance publishes no price for it, so resellers quote it per user per month 2. One vendor's price summary gives about $99 per user per month on a one-year term, $89 on a two-year term and $79 on a three-year term 3. Another adds a one-time implementation fee of $525 during setup, on top of the subscription 4. Ask for a written quote that includes the setup fee and the term.
Accuracy in a 2025 pilot was close, but the sample was small
A 2025 neurosurgery pilot compared a modified Whisper-based model with Dragon Medical One. Dragon Medical One had a mean word error rate of 1.54 percent counting all errors 5, and the Whisper-based model had 1.75 percent 6. The difference was not statistically significant 7.
That is a pilot with three physicians 8 in one specialty, not a verdict on every clinic. Test the product you would buy, with your own clinicians, terms and vocabulary.
HIPAA requires assurances from the vendor before it handles patient information
For health information in the United States, HIPAA's Privacy Rule allows a covered entity to let a business associate create, receive, maintain or transmit protected health information on its behalf only if the entity obtains satisfactory assurance that the associate will safeguard it 9. The Security Rule sets the same condition for electronic protected health information 10. Dragon Medical One's terms of service name a business associate agreement as one of the agreements that govern use of the product 11. The business associate agreement entry explains what that agreement covers.
Whether a dictation app meets that standard for your practice is a question for your compliance team. Ask the vendor for its written terms, check where audio and transcripts are stored and for how long, and confirm whether your data is used to improve the product. The protected health information entry explains what counts.
It depends on your setting, vocabulary and budget
- A single clinician who dictates reports in a quiet room: test a low-cost option against Dragon Medical One on the same reports, and compare the corrections.
- A busy practice with many specialist terms: ask for a trial that uses your own note templates and term list.
- A team that wants notes written from the visit itself: dictation is a different workflow from an ambient medical scribe. See the question on AI scribes and medical dictation for that comparison.
- Any app that uses the public cloud by default: do not enter patient information until the terms are signed.
What to do before you choose
- Write down the 20 terms your clinicians use most, including drug names, procedures and abbreviations.
- Dictate the same 500 words in each candidate, with the microphone your staff will use, in the room where they will work.
- Count the corrections and time the whole job, including editing.
- Get the written privacy and business associate terms before the trial uses real patient information.
- Compare the 12-month cost, including setup, subscription and the time your staff spend correcting text.
