Do vitamins help carpal tunnel syndrome?
The evidence does not show that vitamin B6 (pyridoxine) helps carpal tunnel syndrome. It is often tried for the condition, and the one placebo-controlled trial we found, in 32 people, found no benefit over placebo. Supplements are not a substitute for a splint or a clinician review, and high doses of B6 can cause harm.

No, not as a treatment the trials support. Vitamin B6, also called pyridoxine, is often tried for carpal tunnel syndrome, and its benefit is disputed 1. The one randomized, placebo-controlled trial we found reported no difference from placebo. Older reports are more positive, but they do not settle the question. Taking B6 at ordinary doses is low risk, but high doses can cause harm, so a supplement should not replace a splint or a clinician's review.
A vitamin B6 deficiency theory has not been confirmed
Researchers have proposed that idiopathic carpal tunnel syndrome, meaning the form with no known cause, may be a sign of vitamin B6 deficiency. A 2004 review describes that idea as a hypothesis, not an established cause 2. If the idea were right, a supplement would address the cause. That is why the trials matter more than the theory.
The one placebo-controlled trial found no benefit
In 1993, Spooner and colleagues randomized 32 people with carpal tunnel syndrome to pyridoxine or placebo, and found no difference between the groups in nerve tests, clinical signs or significant symptoms 3. The trial is small, so it cannot rule out a modest effect, but it is the only placebo-controlled trial we found.
Positive reports exist. A 2007 clinical review in Canadian Family Physician states it plainly: although its effectiveness is controversial, vitamin B6 is often used as a conservative and adjunct therapy 1. A 2004 review concluded that some patients improve symptomatically on B6, with low risks of toxicity at recommended doses 4. That is an opinion drawn from earlier studies, not a new trial result.
High doses carry a risk of harm
The 2007 review gives the upper limit for daily vitamin B6 as 100 milligrams 5. The limit is a safety margin. It is not a dose that treats carpal tunnel syndrome.
Mega-doses are a separate concern. A 2004 review of vitamin B6 sold without prescription warns that mega-doses of the vitamin may result in intoxication 6. The article's English abstract gives no dose threshold, so treat that warning as a reason to avoid high doses, not as a safe level.
What to do if you want to try a vitamin
- Do not stop a splint or skip a review because of a supplement. The splint and the clinical assessment are the parts with the most evidence behind them.
- Add up the B6 in every product you take, including multivitamins and combination products, and keep the total under the upper limit unless a clinician advises otherwise.
- Tell your clinician or pharmacist what you take, so they can check it against your other medicines and conditions.
- Judge the result by your symptoms, using a simple log over several weeks, not by how you feel after the first few days.
If your symptoms are getting worse or not going away, a supplement is not the next step. See a clinician. Our guide to carpal tunnel treatment sets out the options in order, starting with the night splint. For the splint itself, see whether to wear a splint at night.
