Question · Dictation on phones and tablets
Is there a medical dictation app for iPhone and Android?
Yes, but for patient information the useful app is usually a vendor tool that works with your electronic health record, not a general phone app. One health system's 2020 Epic setup used Nuance Dragon dictation in the Haiku phone app. A vendor that handles patient information is usually a business associate under HIPAA, and we could not confirm that the phone keyboards meet that requirement.

Yes, there are medical dictation tools for phones, but the one that matters for patient records is usually tied to the record system your clinic already uses. A general dictation app can draft letters and general notes, and our page on a free medical dictation app covers that case. For notes that contain patient information, the question is different: does the vendor agree to protect that information under HIPAA? For the wider comparison of phone dictation options, see our guide to the best dictation apps for iPhone and Android.
The phone app that matters is the one inside your health record
For clinicians, the practical option is a mobile app from the record vendor. A health system's 2020 Epic tips say Haiku on smartphones and Canto on iPads support Nuance Dragon dictation directly into progress notes started in the mobile app 1. The same health system's tips say Dragon Medical One is the tool for dictating into a patient's chart on a desktop or tablet in Hyperspace 2. So which app you can use depends on your employer's setup, not only on your phone.
The figures below are reseller quotes from one pricing summary, not a list price. The summary quotes about $99 per user per month on a 1-year term, $89 on a 2-year term and $79 on a 3-year term 3. Ask your employer whether a license already exists before you buy one.
We did not confirm a current stand-alone Dragon app for iPhone or Android. Check the store listing before you pay for one.
Patient information needs a business associate agreement
Under HIPAA, a vendor that handles patient information for a clinic is usually a business associate. HHS says most vendors and contractors that provide services to covered entities, and that involve access to protected health information, are business associates 4. Our glossary entries on business associate agreements and protected health information explain what the agreement covers.
This is the test to apply to any phone app. If the app sends your speech or text to the vendor's servers, ask whether it offers a business associate agreement before you use it for a patient. Get the answer in writing. Our page on the best medical dictation software takes the same approach across products.
Phone keyboards are a different case. The App Store listing for Gboard on iPhone says that voice input from its microphone is sent to Google for transcription 5. That is a reason to keep patient details out of it. For Apple Dictation, we could not confirm that Apple signs a business associate agreement covering it, so treat it as a general tool unless your compliance team says otherwise.
In a neurosurgery pilot, accuracy was close, but one pilot is not enough
Accuracy matters as much as the contract. In a 2025 neurosurgery pilot, Dragon Medical One had a mean word error rate of 1.54 percent, counting all errors 6. A modified Whisper-based model had 1.75 percent on the same count 7. The pilot found that the difference in mean word error rate across all errors was not statistically significant 8.
Those numbers come from one small pilot in one specialty, so they do not tell you how a tool will do on your notes. Word error rate is the share of words wrong in a transcript, and our glossary entry explains how it is counted. What to do before you dictate patient information
- Ask your clinic or employer which app is approved, and whether it runs on your phone. An approved vendor app is the safest default.
- If the app is not approved, do not enter patient names, dates of birth or record numbers into it.
- Ask the vendor whether it offers a business associate agreement, and get the answer in writing before patient information goes in.
- Test the app on ten of your own notes, and count the errors in each. Use the same test for every app you compare.
- Turn off any cloud or sharing setting you do not need, and keep the app's microphone permission limited to the apps you use it in.
