Question · Dictation on Mac
What is the best medical dictation software for Mac?
We could not confirm a single best medical dictation app for Mac. Check three things before you choose: whether the product runs on a Mac at all, whether the vendor will sign a business associate agreement if it handles patient audio, and whether the audio is processed on your Mac or sent to servers. Accuracy in the one clinical pilot we found was close between Dragon Medical One and a Whisper-based system.

We could not find a best medical dictation app for Mac that we could verify. Dragon Medical One, the product in the 2025 pilot below, is not confirmed as a Mac product, and the decisive question for clinical notes is contractual and technical, not which app has the top score. Ask the vendor three things in writing before you pilot any app: whether it runs on your Mac, whether it will sign a business associate agreement, and where your audio is processed and stored.
Dragon Medical One's Mac support is unconfirmed
Dragon Medical One is Nuance's clinical version of Dragon, and it was the commercial comparison in a 2025 study of neurosurgery dictation 1. In October 2018, Nuance said it had decided to discontinue the Dragon Professional Individual line for Mac 2. That statement covers the consumer Mac product, not Dragon Medical One. The sources we could open for Dragon Medical One did not confirm a Mac version, and some reseller pages that claim one could not be checked. Ask Nuance or your reseller in writing before you plan around it.
HIPAA makes the vendor contract the first test
If a vendor creates, receives, maintains or transmits electronic protected health information on your behalf, the HIPAA Security Rule sets the terms. A covered entity may let a business associate handle that information only if it obtains satisfactory assurances that the business associate will safeguard it 3. In practice, that assurance is usually a signed business associate agreement.
Setup matters as much as the product. Superwhisper's documentation recommends using your own API keys for organizations that already have HIPAA business associate agreements with an AI provider 4. The agreement you need depends on the setup you use, so ask about the setup, not only the product name.
On-device processing narrows the question but does not settle it
Apple says that, for general text Dictation on a Mac, voice inputs and transcripts are processed on your device and not sent to Siri servers, and that you can check this in Keyboard settings 5. We did not find an Apple statement on HIPAA or on business associate agreements for Dictation, so do not treat on-device processing as a compliance decision. Your organization's own policies and risk analysis still apply, whichever app you choose.
Accuracy in the one clinical pilot we found was close
In a 2025 pilot of neurosurgery dictation, Dragon Medical One had a mean word error rate of 1.54 percent, counting all errors 6. A modified Whisper-based model had 1.75 percent on the same count 7. The difference was not statistically significant 8, so the two results are close rather than ranked.
That is one study in one specialty, and its Whisper-based system was a modified model, not the off-the-shelf product. It does not show how any app performs on your notes, your drug names or your accent. Use it as a reason to run your own pilot, not as a ranking.
It depends on your setting and your data
- Hospital or group practice: the institution's IT and compliance teams usually choose the tool, and they will ask for the agreement and the data path first. Ask them which products are already approved.
- Solo clinician on a Mac: a Mac app that keeps processing on the device may limit what leaves your computer, but you still need a policy for stored recordings and for any cloud service.
- Non-clinical dictation of patient information: treat it the same way. The rules apply to the information, not to the job title.
Yap, which we make, processes dictation on the device on Mac and Windows. Put the same questions to us in writing before you use any dictation app for clinical notes, and check our download page for the current platform details.
What to do
- Ask the vendor whether the product runs natively on macOS, in a browser, or not at all, and get the answer in writing.
- Ask for a signed business associate agreement before any patient audio reaches the vendor's servers.
- Ask where audio is processed, how long recordings are kept, and whether they are used to train models.
- Run a pilot with your own dictations, including your abbreviations, drug names and clinic vocabulary, and count the corrections.
- Have your compliance or privacy officer review the setup, including the risk analysis, before clinical use.
