Question · Dictation for work and study
What is the best medical dictation software?
There is no single best product. For speaking straight into an EHR, Dragon Medical One is the product our sources name for EHR dictation; for drafting a note from the visit, compare AI scribes instead; for letters and general notes, a built-in tool may be enough. Test each shortlisted product on ten of your own notes, and get a business associate agreement before any patient data goes in.

The best medical dictation software is the one that matches how your notes get written and checked. Three different workflows are sold under the same name, so a product that is best for one clinician can be a poor fit for another. Start by deciding who corrects the words, then compare products inside that category. Our guide to medical dictation explains the three workflows in full.
Dragon Medical One is the product our sources name for dictating into the chart
For a clinician who wants to speak directly into the electronic health record, Dragon Medical One is the product our sources name for that job, though that is a short list. One health system's Epic tips call it "the app to use when dictating into a patient's chart on a desktop or tablet in Hyperspace" 1. That quote is a 2020 health system tip, not an independent test. Check that the version you are buying supports your EHR and your workstation. Ask the vendor which product a quote covers: desktop dictation, the ambient assistant, or both.
Accuracy evidence is thin, and it does not separate the leading products
A 2025 pilot in a neurosurgical practice compared Dragon Medical One with a modified Whisper-based model. Dragon Medical One had a mean word error rate of 1.54 percent, counting all errors 2. The Whisper-based model had 1.75 percent 3. The difference was not statistically significant (p = 0.080) 4. The pilot covered one specialty, so the result says little about yours. The honest reading is that accuracy alone did not decide between them in that pilot.
Our question on how accurate voice recognition is explains why a single accuracy number rarely transfers from one setting to another.
Price depends on the term, so get an itemized quote
Resellers quote Dragon Medical One per user per month, with lower rates on longer terms. The rates we found run from $99 per user per month on a one-year term to $79 on a three-year term 5. Our question on what Dragon Medical One costs works through the totals.
AI scribes are a different category, and their results vary
An AI scribe listens to the visit and drafts the note afterwards, so the clinician does not dictate at all. Compare these with dictation products only when the job is writing the note from the conversation. In a 2025 randomized trial, physicians who used Nabla cut their time in the note by 9.5 percent against the control group, the only arm with a statistically significant reduction 6. Results differ between products, so test before you buy. Our comparison of AI scribes and medical dictation covers the choice in more detail.
A general tool can be enough for letters and general notes
For correspondence and notes that contain no patient detail, a free built-in tool may be enough. On a Mac, Apple says general Dictation voice inputs and transcripts are processed on your device and not sent to Siri servers 7. Voice Control on a Mac can hold custom vocabulary terms, but it is a general-purpose tool and not a clinical vocabulary. Our question on free medical dictation apps covers the free options in more detail.
Yap, which we make, is a general dictation app for Mac and Windows. It is not built for clinical documentation, so we do not rank it against EHR products here.
Patient data changes the decision before features do
If a tool will receive patient information, the cloud provider generally needs a business associate agreement. HHS says a covered entity and a cloud service provider that holds its health information must sign a HIPAA-compliant business associate agreement 8. On-device processing reduces where audio travels, but it is not a substitute for that agreement or for your own risk analysis. A product you cannot get a signed agreement for is not a candidate for clinical notes, however accurate its demo.
What to do: a four-step selection
- Pick the workflow. Decide whether you will dictate into the chart, send audio to transcription, or use an AI scribe.
- Shortlist two or three products that support that workflow on your EHR and your devices.
- Dictate ten real notes from your specialty with each product, count the errors, and compare the results. Use the word error rate as the measure.
- Get the paperwork first. Obtain a signed business associate agreement, an itemized quote with the full term, and confirmation of the EHR integration before any patient data goes in.
