What is the difference between Dragon Medical One and Dragon NaturallySpeaking?
Dragon Medical One is Nuance's clinical speech recognition product, sold per clinician as a cloud subscription and built to write into health records. Dragon NaturallySpeaking is the general desktop product for ordinary documents, sold as a license for one user. Dragon Medical Practice Edition, the older medical desktop product, reached end of life in 2021, so it is not a current option for clinical notes.

Dragon Medical One and Dragon NaturallySpeaking come from the same speech recognition company, but they are sold for different work. Dragon Medical One is for clinicians who dictate notes into health records. Dragon NaturallySpeaking is for general documents. Choosing the wrong one means paying for features you do not need, or lacking the record integration you do need.
The names have changed over time, and Microsoft's Dragon Copilot now combines and extends the clinical product. Our guide to medical dictation covers the workflows, and this question covers the product difference.
Dragon Medical One is a cloud subscription built for clinical notes
Philips, which resells the product, describes Dragon Medical One as "Cloud-based clinical speech recognition for documenting care in the EPR and beyond" 1. The product is sold per user 2, and Philips says it needs no voice profile training, because a single cloud profile is set up at first use 3. Both are the vendor's claims, so test them on your own speakers.
The clinical focus shows in the record integration. One health system's Epic tips name Dragon Medical One as the tool for dictating into a patient's chart on a desktop or tablet in Hyperspace 4.
Microsoft now includes it in a wider product. Microsoft's Dragon Copilot combines and extends Dragon Medical One and DAX Copilot 5. A quote may use either name, so ask which product it covers.
Dragon NaturallySpeaking is a general desktop product
Dragon NaturallySpeaking is the general product, sold for ordinary documents. The desktop line is sold as Dragon Professional, and a US reseller lists a one-user Dragon Professional v16 license at $685.99 6. That is a license for one person, not a per-clinician subscription.
Cloud versions exist in the general line too. A US reseller describes Dragon Professional Anywhere as cloud-hosted speech recognition for the enterprise 7. So "cloud" alone does not tell you that a product is medical. Check the product name and what it is sold for.
The medical edition of NaturallySpeaking has been retired
Dragon has had a medical version of the desktop product before. In a 2000 study, physicians dictating medical reports with Dragon NaturallySpeaking Medical Suite 3.0 had a vocabulary error rate of 14.1 to 15.2 percent 8. Those numbers describe software from the late 1990s, so they tell you about the history of medical speech recognition, not about today's products.
The later medical desktop product was Dragon Medical Practice Edition. Philips says Nuance's Dragon Medical Practice Edition reached end of life on December 31, 2021, and would no longer get updates or work with new operating systems 9. Regional notices we saw gave other dates, so check the notice for your country.
No official free download of Dragon NaturallySpeaking Medical 10.1 turned up
We did not find an official free download of Dragon NaturallySpeaking Medical 10.1, and we found no published support dates for that version. It is a legacy release, and the medical desktop line has been retired, so we could not find a current supported version to download. Files offered on unofficial download sites are a risk: they may be unlicensed, and they may carry malware. If you hold a license for an older Dragon product, contact Nuance or your reseller about what it still allows.
Which product fits the work

|---|---|---|
| Notes typed into an EHR | Built for it, and named in one health system's Epic tips | Not the product those tips name |
| How it is bought | Per clinician subscription through resellers | One-user license, or a cloud version |
| Where it runs | Cloud service | Desktop software |
The table reflects what the sources above state. If a vendor tells you otherwise, ask for the claim in writing.
What to do
- Name the product on the quote. Ask whether it is Dragon Medical One, Dragon Copilot or a general Dragon license.
- Check the record connection. Ask whether the product types into your health record on the devices you use.
- Compare the cost on the same basis. Per-clinician subscription and one-user license are different purchases. For the subscription figures, see the question on how much Dragon Medical One costs.
- Read the general comparison if you are not clinical. For ordinary documents, see Dragon Dictate vs Dragon NaturallySpeaking and the question on whether Dragon NaturallySpeaking is discontinued.
