What it measures
45 CFR 160.103 is the definitions section of the HIPAA Administrative Simplification rules, and it applies to the Privacy and Security Rules that cover health care providers, health plans and clearinghouses, and their business associates.
What it does not show
The definition does not say which data counts as individually identifiable. That is set by the separate identifier rules in the Privacy Rule, and whether a particular dictation app handles PHI depends on the facts of its use.
